Polk County Housing Continuum

The Polk County Housing Continuum (PCHC) is a comprehensive group of sheltering and housing non-profit organizations, private funders and governmental entities that develop and implement strategies to address the needs of homeless, near homeless and low income households in our community. The focus of the PCHC is to ensure that all families and individuals have safe and affordable housing. These agencies have been working diligently over the past few years to define the services and housing needs within our community, and develop and implement effective strategies to meet those needs.

Des Moines Homeless Action Plan

The City of Des Moines in partnership with Polk County, United Way of Central Iowa, Des Moines Community Foundation, and the Polk County Housing Trust Fund are creating an Action Plan to address the needs of the homeless population in our community.
